How to Request Your California Driving Record

You can get a copy of your California driving record online, in person, or by mail.

Requesting your driving record online is simple, but it is not an official document.

To request a copy in person, make an appointment at your local California DMV office for faster service.  You must complete the driving record request form, show valid ID, and pay a fee.

To request your California driving record by mail, send the completed driving record request form, with a check or money order for $5, to:

California DMV
Office of Information Services
Public Operations Unit G199
PO Box 944247
Sacramento, CA  94244-2470

More information on requesting your California driving record may be found on the California DMV website.
