How to Request Your Illinois Driving Record

You can get a copy of your Illinois driving record (called a Driving Record Abstract) in person or by mail by completing the Abstract Request Form and paying a $12 fee.  

If you are mailing your request, include your name, DOB, sex, and drivers license number, and send to:
