How to Request Your New York Driving Record
New York offers many options for requesting your New York driving record. Detailed instructions follow.
Online - Immediate Download
This is the fastest way to get a copy of your driving record. You must register (if you haven't already) at NYS MyDMV (https://my.dmv.ny.gov/crm/), and will need to provide your drivers license and social security numbers. Your record will be available for immediate download (as a pdf).
Online - Driving Record Mailed within 2 Weeks
If you prefer NOT to register at NYS MyDMV, you can still make your request online (https://transact.dmv.ny.gov/PersonalAbstracts/), but your record will be mailed to you usually within two weeks.
By Mail
To request your driving record by mail, complete the NY Request for DMV Records form and send it to the address on the form, along with a $10 check or money order ( payable to the Commissioner of Motor Vehicles) and a copy of your drivers license.
By Phone
Requesting your driving record by phone is more time-consuming than online, but it is possible. Simply call the DMV Call Center at

In Person
Take a completed form MV-15C (NY DMV Request for Driving Record Information) and $10 to one of the many NY DMV offices.
