Instructions for Obtaining a West Virginia Driving Record

You may request a copy of your West Virginia driving record in person at any West Virginia DMV branch office, by mail, or through email or fax (a follow up phone call is required for payment).

You must provide the following information with your request:

  • Completed Driving Record Request Form
  • Copy of state issued photo ID
  • Processing Fee:
    • $5.00 if you provide name and driver's license number
    • $6.00 if you don't have driver's license number (you must provide DOB and/or SSN)
  • The address where the form should be mailed
